Terms for embedding the assessment
Healthcare websites may embed the RejuvaDisc Back Pain Assessment free of charge using the provided code, subject to these practical terms.
What the widget does
The assessment provides general educational information. It is not a diagnosis, does not replace professional advice, and must not be presented as an emergency service. Keep urgent warning and emergency guidance visible. For urgent or life-threatening symptoms, visitors should use local emergency services.
Who provides the service
The assessment and its consultation CTA are provided by RejuvaDisc. Do not describe the consultation as a service of, or endorsement by, the host clinic unless that is separately and accurately agreed. The host is responsible for its own site content, accessibility, privacy notice, and compliance obligations.
Installation and attribution
Do not modify the widget in a way that misrepresents its source, clinical purpose, or safety information. The embed generator includes a visible “Powered by RejuvaDisc” link beneath the widget by default; you may remove it or add a suitable link relationship attribute. When the external link is enabled, the widget's internal credit is hidden to avoid duplicate branding. Do not use hidden links or misleading endorsement claims.
Availability and data
The widget is offered as-is and may change or be unavailable. The host page sends its hostname and page path to support anonymous usage reporting. Do not place the widget on pages whose URL path contains sensitive personal information. See the privacy notice for processing details.
