Privacy information for the assessment
The RejuvaDisc assessment is an educational symptom conversation. It does not ask for your name or contact details. Please do not include information that identifies you.
How your answers are processed
Your answers are processed by the assessment service to determine the next question and result. When the conversational service is enabled, the current answer and limited recent assessment context are sent from our server to Ollama Cloud to help interpret the response. They are not sent directly from your browser to Ollama. Do not use the tool if you do not want symptom information processed this way.
Analytics
We may record anonymous assessment events such as a start, completion, or consultation CTA interaction. We do not send raw symptom answers, names, phone numbers, or full conversation transcripts to the analytics pipeline. For embedded visits, the partner website hostname and a sanitised page path may be recorded to understand where the widget is used. These values are supplied by the embedding page and are reporting metadata only, not a security signal. We do not use cross-site tracking or fingerprinting.
Telephone consultations
If you choose to contact RejuvaDisc, your phone app opens with our consultation number. The assessment does not place a call automatically. Any information you share during a call is handled as part of that separate contact.
External websites
A clinic or website embedding the assessment controls its own website and privacy practices. RejuvaDisc provides the assessment and any RejuvaDisc consultation; the host website does not provide or endorse those services unless it says so separately.
Optional clinic logo
An embedding website may choose to supply a public HTTPS logo image URL. If supplied, each visitor’s browser loads that image directly from the URL’s host, without sending a referrer header. Do not use a URL containing private or temporary access credentials.
